Testing Program: Getting Started Checklist
The following checklist will help you get started with testing through the New Mexico Schools COVID-19 Testing Program.
- Review the NM Schools COVID-19 Testing Program Options, and select the testing program(s)* that best fit your needs:
- Program Option A: Full-Service Testing with PMG
- Program Option B: COVID-19 Test Training with PMG
- Program Option C: School-Administered Rapid Antigen Testing Kits
- Understand program requirements and next steps: visit our School Resources page, CLIA Certificate of Waiver 101, and FAQs for more information.
- Contact us with questions or attend Office Hours.
- Submit a registration form.
- Registration must be completed by the individual authorized to sign and accept funding assurances (i.e., Superintendent, Charter School Director)
* Direct Funding (Program Option D) is entirely separate from your selection of other programs. Opting into Program Options A through C does not impact your total funding amount. However, funds may not be used to purchase testing kits or contract with testing providers.
- Receive your program confirmation email, review all attachments, and complete next step instructions
- If you selected Full-Service Testing with PMG ( Program Option A) and/or COVID-19 Test Training with PMG (Program Option B):
- Watch our introduction webinar with Premier Medical Group.
- Review the sample paper registration and consent forms (English and Spanish) and determine if any changes are needed.
- Complete the Orientation worksheet.
- Contact us with questions or attend Office Hours.
- Respond to request from our team to schedule a regional logistics orientation.
- If you selected COVID-19 Test Training (Program Option B) and/or School-Administered Rapid Antigen Testing Kits (Program Option C):
- Apply for your CLIA Certificate of Waiver
- Register with SimpleReports for mandatory test reporting
- Other Recommendations for Site Preparedness:
- Ensure you have enough Personal Protective Equipment (PPE)
- Review your communication, consent and notification processes. You must use the consent forms provided by the program for Program Options A, B, and C.
- Ensure you have a mechanism to safely dispose of medical waste
- Review your plan for safe isolation and quarantine
- If you selected Testing and/or Training with PMG (Program Option A and/or B):
- Once you have determined preferred dates, locations, and testing methods, you may schedule your implementation meeting at this link: https://calendly.com/new-mexico-schools-covid-19-testing-program.
- If you are participating in the School-Administered Rapid Antigen Testing Kits (Program Option C):
- Submit the following to info@nmschoolscovidtesting.com (if you haven't already)
- Proof of your CLIA Certificate of Waiver
- Confirmation that testing staff will watch the provided training
- Confirmation that you have registered in SimpleReport
- Once you receive the standing order and the test kit order forms, limit your test supply orders to what you need for one month
