Advance Balance Status Notification
Form Instructions
The purpose of this form is to ensure that we know the status of the advance balance for your school or school district.
Please complete the form as accurately as possible.
Additional Instructions
If the advance payment was already expended on allowable funds, please wait for instruction from NMDOH for where to submit your reports.
If you have received preapproval for other expenses through our website, our expectation is that those expenses are paid from your advance payment.
If your school or district is not planning to spend the advance balance on allowable funds, please process the invoice sent to you via email and mail no later than January 6, 2023 so that we may return the funds back to the state. If you are not able to process funds that quickly, please notify us of your intention prior to the deadline date.
